We all hope that when we schedule an appointment for major medical procedures that we have picked the right doctor and facility to ensure the best possible outcomes.  But, we also hope that the doctor we pick is not only great surgically, but also has a demeanor that makes us feel comfortable and in the “right hands” or what is often known as bedside manner.  Read the testimonial below given to me by Mark Smith a patient of Dr. Barrington’s and I think you’ll be as excited to schedule an appointment with him as we are that he’s joined our practice.

Dear Plano Orthopedics,

Perhaps the most compelling thing I can say speaks to the outcome!  The months prior to my knee replacement surgery I was bypassing stairs and using wheel chair ramps instead.  Then 358 days after my surgery, I summited two 14,000+ foot peaks in Colorado with my 17-year old son.  I also climbed another peek the next morning while my son slept. 

From our first meeting with Dr. Barrington I knew that I was dealing with someone special.  He is extremely knowledgeable, careful to explain, patiently answered all of my questions and ensured that I understood and was comfortable with the work that would be done.  I also found him to be very humble and sensitive to our needs.  That first experience was so exceptional that my wife nearly started crying because the appointment was such a departure from other specialists we had seen.

He also demonstrated his deep care for patients when he spoke a blessing over me just prior to the surgery:  “May the Lord bless you and keep you, and make his face shine on you, and be gracious to you, and give you peace.” Those are pretty comforting words from the Bible no matter your faith experience.

How are PAs educated and trained?

They then complete a total of more than 2,000 hours of clinical rotations in:

In order to maintain certification, PAs must:

  • Complete a recertification exam every 10 years

Pacira Announces Data Reinforcing Benefits of EXPAREL® for Postsurgical Pain Control Following Total Hip and Knee Replacement Surgery

— Results Presented at the 2015 Annual Meeting of the American Academy of Orthopedic Surgeons Find Overall Hospital Cost Savings of More Than $1.5 Million —

PARSIPPANY, N.J., March 26, 2015 – Pacira Pharmaceuticals, Inc. (NASDAQ: PCRX) today announced data demonstrating the benefits associated with the use of EXPAREL® (bupivacaine liposome injectable suspension) in a study of over 2,200 hip and knee replacement patients. According to the analysis, patients treated with EXPAREL reported significantly lower pain scores, were more likely to report having “zero pain” during their hospital stay, and were associated with lower hospital costs than patients treated with the standard pain management regimen. The results were presented at an oral podium session at the 2015 annual meeting of the American Academy of Orthopedic Surgeons (AAOS), being held in Las Vegas March 24-28th.

More than 1.1 million total joint arthroplasties (commonly known as joint replacements) are performed annually in the United States, and postsurgical pain is typically managed with continuous regional nerve blocks and systemic opioids, which can be associated with unwanted side effects such as leg weakness and sedation, both of which increase the risk of falls. Periarticular injections (PAI) of pain medications have been shown to be an effective approach to control postsurgical pain while improving function and reducing the need for opioids.

This analysis, which builds on a smaller subset of data presented at the 2014 AAOS annual meeting, evaluated 2,248 patients who underwent hip or knee arthroplasty and received a standard well-established multimodal regimen including PAI. Half of the patients received PAI with bupivacaine HCl, with or without ketorolac, and morphine, while the other half received PAI with EXPAREL. The primary outcomes measured were Visual Analog Scale (VAS) pain scores and the percent of VAS pain scores that were 0, meaning patients reported “no pain.” In addition, length of stay, patient-reported satisfaction and per patient costs for supplies and pharmaceuticals were compared between the two treatment groups.

Key findings associated with the use of EXPAREL included:
• Significantly lower pain scores (1.98 vs. 2.43, P<0.0001)
• Greater percentage of patients reporting “no pain” for hip (57.3% vs. 43.4%, P<0.0001) and for knee (47.2% vs. 42.1%, P <0.0001)
• Decreased length of stay for patients undergoing knee replacement (2.40 days vs. 2.69 days, P<0.001)
• Improved Press Ganey overall satisfaction (98.3% vs. 96.7%; P=0.0221)
• Average cost savings of $1,246 per patient, which resulted in an overall hospital savings of over $1.5 million, predominately attributed to eliminating the need for femoral nerve catheters, knee immobilizers, and patient controlled analgesia (PCA) pumps to deliver IV opioids

“Getting patients on their feet and moving is critical to optimizing long-term treatment outcomes after hip or knee surgery, and the ability to provide a single-dose long-lasting alternative to opioids and catheter-based regional nerve blocks—which can negatively impact ambulation and the patient recovery experience—is a major advantage for orthopedic patients,” said John Barrington, M.D., orthopedic surgeon at Plano Orthopedic Sports Medicine & Spine Center, Plano, TX. “Our analysis found that the use of EXPAREL can improve both pain control and patient satisfaction, while resulting in a meaningful cost savings per patient.”

“Dr. Barrington’s data reinforces a large and growing body of clinical and real-world evidence supporting the utility of EXPAREL in joint replacement procedures,” said Dave Stack, president, chief executive officer, chairman and director of Pacira. “In addition to improving key outcomes such as pain scores, length of stay, and costs of care, EXPAREL provides the added value of reducing the need for opioids and thereby minimizing unnecessary exposure to potentially serious downstream consequences like long-term use and abuse, especially important in the context of the opioid epidemic in the United States today.”

EXPAREL is currently indicated for single-dose infiltration into the surgical site to produce postsurgical analgesia. The product combines bupivacaine with DepoFoam®, a proven product delivery technology that delivers medication over a desired time period. EXPAREL represents the first and only multivesicular liposome local anesthetic that can be utilized in the peri- or postsurgical setting. By utilizing the DepoFoam platform, a single dose of EXPAREL delivers bupivacaine over time, providing significant reductions in cumulative pain scores with a 45 percent decrease in opioid consumption; the clinical benefit of the opioid reduction was not demonstrated. Important Safety Information
EXPAREL is contraindicated in obstetrical paracervical block anesthesia. EXPAREL has not been studied for use in patients younger than 18 years of age. Non-bupivacaine-based local anesthetics, including lidocaine, may cause an immediate release of bupivacaine from EXPAREL if administered together locally. The administration of EXPAREL may follow the administration of lidocaine after a delay of 20 minutes or more. Other formulations of bupivacaine should not be administered within 96 hours following administration of EXPAREL. Monitoring of cardiovascular and neurological status, as well as vital signs should be performed during and after injection of EXPAREL as with other local anesthetic products. Because amide-type local anesthetics, such as bupivacaine, are metabolized by the liver, EXPAREL should be used cautiously in patients with hepatic disease. Patients with severe hepatic disease, because of their inability to metabolize local anesthetics normally, are at a greater risk of developing toxic plasma concentrations. In clinical trials, the most common adverse reactions (incidence greater-than or equal to 10%) following EXPAREL administration were nausea, constipation, and vomiting.

DO I NEED A KNEE REPLACEMENT?
How many Americans have arthritis of the knee?

With people living longer than ever, arthritis of the knee is becoming more common. There may be a need for 3,000,000 knee replacements each year by the year 2030.

How effective are total knee replacements at treating arthritis?

Treatment of arthritis starts without surgery. Over-the-counter pain relievers and anti-inflammatory medication may help. Using a cane or avoiding doing things that hurt may give relief as well. But you may develop pain that can only be treated by surgery. At first, you may only have pain or stiffness when walking a long way. As the arthritis gets worse, routines like taking short walks, putting on shoes, or dressing may cause pain. Arthritis of the hip and knee can affect your life in many ways – including how you feel psychologically.

The good news is that knee replacements are very successful surgeries. It takes time to heal afterwards, but many people return to an active, pain-free life. Less pain usually leads to better walking ability and improvement in your overall health.

Is it worth the expense?

A common way to measure the value of a procedure is to compare the cost to the quality years of life it gives a person. The cost of the surgery itself is high, but the improvement to quality of life is great and sustained; thus, the overall costs in general are considered low. Your general health and sense of well-being also gets better. Nine out of ten people say they would have the same surgery again to treat their arthritis.

When is the right time to have my joint replaced?

The right time for joint replacement surgery is a common concern. Many factors are important to think about: general health, time away from work, family commitments, and the time it will take you to get better afterwards. Most people decide the time is right when their knee pain prevents them from living comfortably. In many cases, arthritis pain will prevent you from doing very simple things. Perhaps you cannot take care of your home or family, or you can no longer do your job. You must make the individual decision about the right time to have surgery.

Is there a problem with waiting too long before deciding to have your knee replaced?

People with knee arthritis have disability from two things: Pain and Mechanical symptoms such as locking of the joint. Some people suffer from pain, swelling, and stiffness for years before considering surgery. Other people see a doctor when mechanical symptoms (buckling, clicking, grinding, or limping) get worse. These symptoms can jeopardize safety at home or at work. As knee arthritis worsens, the stiffness of the arthritic joints also worsens. This can make the replacement surgery more difficult. That may mean a longer recovery and more physical therapy. Unfortunately in severe cases, joint flexibility may never return to normal. By waiting too long, you may not get the full benefits of your knee replacement surgery.

How long will I stay in the hospital?

You will likely stay in the hospital for one to three days depending on your rehabilitation protocol and how fast you progress with physical therapy. This is highly dependent upon your condition before surgery, your age, and medical problems which can hinder your rehabilitation.

What is recovery like in the hospital?

Recovery starts right after surgery. You are helped out of bed on the day of or the day after surgery. A physical therapist will help you to walk. Most patients will have one or two sessions of physical therapy per day. The goal of therapy is to assist with strengthening of the muscles and walking. Therapy will also make sure that you are safe when you go home. That’s important when doing things like dressing, using the bathroom, getting up from a chair, and climbing stairs.

Walking soon after surgery helps you get better. It also helps avoid things like bedsores, pneumonia, and blood clots. While moving around helps prevent blood clots, most doctors will use a more formal program of blood clot prevention like stockings worn on the legs, inflating foot or leg pumps, and blood thinning medications. These medications may be continued after you go home.

Will I be in a lot of pain?

Fear of pain from surgery is one of the biggest reasons why people avoid having a knee replacement. With better pain control, you will have mild to moderate pain. Pain control comes from using several medications that affect both the spinal cord and the brain. Doing so means smaller doses and fewer side effects like nausea. Surgeons may also inject pain medicine into the knee at the time of surgery to numb the area. At many hospitals, pain medications are given even before surgery begins.

Nausea can make recovery harder. It has many causes including stress and pain medications. Using less medication that includes narcotics (like morphine) will help to lessen nausea. There are also medications that help control nausea if it occurs.

When will I be back to normal?

Most people get better from knee replacement by six weeks. The skin incision or cut takes approximately two to three weeks to heal. The time it takes to walk without a cane or drive after surgery is different for everyone. You will need physical therapy after going home. Even though the skin incision or cut will heal in two to three weeks, the process of healing can take up to a year. Scar tissue tends to soften over time, so you will continue to improve even after your physical therapy is over.

Studies show that about eight out of ten people who have knee replacement are pain-free within a year. Once you are without pain, you will notice an improvement in your ability to walk. A new knee may allow you to return to your favorite pastimes like walking, swimming, gardening and even some low-impact sports.

How long will my new joint last?

For about nine out of every ten people who have had a knee joint replaced, the new joint is still working well after twenty years. How long the replacement will last depends on a number of things. Younger individuals who are more active tend to wear out their replaced joints quicker. Older, less active individuals find their joint replacements last longer.

Will I need follow-up care after I’m well?

Replacements may fail by the parts becoming loose. The joint surfaces may wear. Bone could break down around the parts, infection could set in, or in rare cases, the parts themselves might break. Many of these problems can be seen by a doctor on x-rays before you feel that anything is wrong. This is why you should see your doctor on a regular basis after surgery even if you feel well. Treatment soon after a problem occurs is usually simple. But if the problem is ignored, it can be much harder to fix.

Dear Plano Orthopedics,

I must say that my partial knee replacement with Dr. Barrington was a very positive experience from start to finish and very successful.  Dr. Barrington, his staff, the physical therapist and the hospital staff each performed flawlessly.  Dr. Barrington was accurate in his assessment of what was causing my knee pain, predicting a partial knee replacement, but said he was prepared for a full replacement if he saw the need once he opened  up the knee.  I only needed a partial.

The operation started at 1:00 pm and ended about 45 minutes later.  I went home that evening at 8:30 pm after demonstrating that I could get out of bed and walk (with a walker).  There was little-to-no pain.  After the surgery, I had only one day (the third) when the pain required a pain pill.  After that, any pain was minimal.  I used a walker for two to three weeks following a surgery, mostly as a precaution.  After that, I could walk without a walker or a cane.

I had a physical therapy three times a week.  This seemed to be a key to my quick recovery-got the joint moving to help re-establish a good range of motion and strengthen the muscles while being careful not to overdo.  The physical therapist who worked with Dr. Barrington, Atefeh Hajialikhani, was great in providing the direction and encouragement for the physical rehabilitation.

After 4 weeks, I was free to drive, and after 5 weeks my wife and I were on the dance floor once again, but this time completely pain free.  Patient Testimonial 2 copyAfter week 6, we went to a dance event held at Purdue University for 6 days of dancing-morning, afternoon and night-learning ballroom dance routines by day and dancing at night.  Yes, we were tired and our leg muscles were sore, but not even once did I experience any knee pain.  What a contrast to what I was feeling before the operation!  Not bad for a 70 year old.

I attribute this outstanding success to three factors – the surgical expertise of Dr. Barrington, the scripted rehabilitation process established by Dr. Barrington and executed by Atefeh, and prayer.  We could not be more pleased with the outcome!

We are so thankful that after years of pain we found Dr. Barrington.  Choreographed ballroom dancing has been a very important part of my life and my wife’s life.  For the past few years we were sitting out more than we were dancing.  We were even trying to accept the fact that we would have to quit dancing.

Dr. Barrington gave us an important part of our life back.  Six weeks after surgery we were able to attend a six day dance event with all day workshops and dancing in the evening.  No one could believe we could keep up!

The expertise and incredible care I received were remarkable!  Friends even notice the happy expression on my face now compared to the grimace of pain when trying to dance in the past.  Perhaps an overused expression, but I sincerely feel I have a new lease on life!  I have absolutely no knee pain.

I wholeheartedly recommend Dr. Barrington to anyone considering knee surgery.

John W. Barrington, Plano Orthopedic Sports Medicine & Spine Center Joint Replacement Surgeon and co-director of the Joint Replacement Center at Baylor Medical Center of Frisco, presents information regarding Pain Control, Injections and Outpatient Knee Arthroplasty, or Total Knee Replacement: Where we are in 2015.  Dr. Barrington discusses the different ways in which he uses pain medications and how he aims to treat his total knee replacement patients.  The information provided is very detailed, however, very interesting to hear our physicians point of view and how he thinks about his knee replacement cases and where we are medically in 2015.  This slideshow presentation was presented at the 2015 Annual Surgical Pain Consortium in Dallas, TX.

Currently, Dr. Barrington’s patients are 98% same day or next day candidates.  What does this mean?  Currently, 40% of his patients are going straight home following outpatient surgery for joint replacement of the knee.  This is a innovative approach to knee replacment surgery as opposed to the typically model of staying in the hospital as an inpatient following your procedure.  Dr. Barrington follows a protocol or guidelines to determine if you are a candidate for same day or next day knee replacement surgery.

Plano Orthopedic Sports Medicine & Spine Center is showing off their dance moves in celebration of Super Bowl 50 POSMC Style End Zone Dance 2016.  Our POSMC employees pick their Super Bowl team, this year the Broncos vs Panthers, and we enjoy a number of employee events throughout the week to gain points.  Each time gets prizes for individual events and of course a grand prize and pizza party for the winning team!

Our 2nd Annual POSMC Super Bowl End Zone Dance Challenge has been another fun one to watch.  Each team has 2 contestants, they must dance and get the most Facebook likes on our www.facebook.com/posmc page to win the “Dance”!

Dr. Randal Troop is a certified and fellowship trained orthopedic surgeon with Plano Orthopedic Sports Medicine & Spine Center.  Dr. Troop specializes in sports medicine injuries and is currently the head orthopedic physician for the Frisco, TX RoughRiders and an orthopedic consultant for the Texas Rangers as he travels with the team during spring training and consults injured players throughout the season.  He has worked as the team physician for multiple local high schools and colleges and continues to be recognized as a top sports medicine doctor in his orthopedic field.

Dr. Randal Troop was recently interviewed and published in Frisco Style Magazine January 2016 Health and Fitness issue discussing sports injuries in kids and teens.  The article titled, “It’s All Fun and Games Until…” discusses the strong presence of athletics in Frisco, TX and how more young children are experiencing sports injuries today due to participating in high-level sports at such a young age.  Dr. Troop believes the increase in injuries is associated with, “children playing sports all year round and also practicing and playing multiple hours every day.  Both of these activities can lead to increased stress of growing bones and joints in young children, which results in injury.” Dr. Randal Troop talks sports injuries in kids and teens

Dr. Randal Troop’s experience with a variety of sports injuries in kids and teens has lead him to believe they are most commonly divided into two categories, Traumatic Injuries and Overuse Injuries.  Traumatic injuries, “such as broken bones and torn ligaments are often difficult to avoid because they are part of playing sports.”  Dr. Troop continues, “however, you can decrease the incidence of traumatic injuries such as concussions or broken bones by limiting or avoiding contact sports such as football, hockey and soccer.  These types of injuries can occur in non-contact sports such as baseball, gymnastics and basketball though not as commonly seen.  Traumatic injuries can often be avoided by proper equipment such as helmets or making sure your coach is teaching proper techniques for the sport in question.” Dr. Randal Troop talks sports injuries in kids and teens

Overuse injuries often seen with too much running, jumping, dancing or throwing “are much more under the control of the coaches and parents,” says Dr. Troop.  “Overuse injuries occur when young athletes train too many hours in a week or are focusing on multiple sports every day.  Children’s growing bodies need time to heal themselves and the repetitive strain on their bones and joints with multiple hours of practice or sports aren’t allowing enough recovery time to prevent overuse injuries.  This continued stress on the body can lead to stress fractures, growth plate injuries and tendinitis.”

So what can parents do to help their kids and teens prevent sports injuries, but still enjoy the wide community of sports in Frisco, TX?  Dr. Troop recommends, “monitoring the amount of practice time and games allowed per week and parents should be vigilant in listening to their children when they begin to complain about pains they are having during or after practice and games.”  Dr. Troop continues to mention, “soreness of course is normal with playing and practicing sports, however, children who continue to play and complain of pain with activities should not be encouraged to push through it.”  Adults have a higher capacity for working through soreness and pain, children’s growing bones and ligaments are not as strong and may not be able to withstand the repetitive trauma of playing day after day. Dr. Troop shares a story of a patient who was a teenage boy playing basketball and complaining of prolonged knee pain.  His diagnosis was an injury to his growth plate and Dr. Troop recommended 4-6 weeks rest until the pain resolved and the injury healed before continuing basketball.  The patient had a big tournament he didn’t want to miss and decided to play regardless of Dr. Troop’s recommendation and ended up pulling off the growth plate in his knee requiring surgery.   He then missed 6 months of basketball following surgery with Dr. Troop and the rehabilitation of the knee as opposed to 4-6 weeks.  Dr. Troop believes, “the main job of the parent is to be the advocate for their child as they navigate through the large world of sports.  Often times coaches can be short sighted and not realize the stress they are putting on growing bones and joints.  Coaches often may not be aware of the many sports their athletes are participating in as well.  Parents have the final responsibility in deciding when their children are doing too much and when this can lead to an overuse injury.”

A good rule of thumb is to rest, utilizing “RICE” method when children are complaining of injury or soreness.  R-rest, I-ice, C-compression, E-elevation depending on the injury and of course NSAID’s such as Advil to assist with pain and swelling.  Take a small break from the kids and teens overactive schedule and see if the pain subsides.  Dr. Troop advises parents, “to have pain evaluated by a trained sports medicine specialist if their child complains of increasing pain that does not resolve with the rest and Advil recommended above.  He urges parents not to encourage children to push through the pain and to listen to what their kids and teens are saying about their sports injury.” Dr. Randal Troop talks sports injuries in kids and teens

“Bottom line is that the impact of sports injuries can be long-lasting or detrimental to the future plans of a young athlete.  Taking control and knowing when to seek medical attention is an easy way to keep your athlete in the game, off the bench and performing at their highest level.” says Allie Spletter, a Frisco Teacher and Coach.

Dr. Troop talked with Steve Eager with Fox 4 News about the upcoming Chalk Talk Event with Baylor Medical Center of Frisco and Sci-Tech Discovery Center regarding the Top 5 Sports Injuries in kids.

One of the top injuries Dr. Troop is seeing in kids is overuse injuries.  Kids are playing sports year round and in addition to playing one sport, they are often playing multiple sports and having private lessons.  It’s important that the parents are their kids advocates and constantly monitoring and watching how they are reacting to sports and injuries.

Chalk Talk Event is a 3 part series being held at Sci-Tech Discovery Center in Frisco with Baylor Medical Center of Frisco geared to help educate our parents and kids about the proper training, conditioning and injury prevention needed to maintain a fun and healthy athletic environment.

Dr. Barrington Hip Replacement Mission TripJohn W. Barrington, MD travels to the Dominican Republic in the spring for a Mission Trip with MOVE Missions.

Dr. Barrington Hip Replacement Mission Trip.  John W. Barrington, MD and his team travel to the Dominican Republic to provide healthcare and surgery for patients who are either too poor to afford the care, they have no opportunities to receive this kind of medical care, or the cases are too hard for the medical care providers in that area.

The MOVE mission does their best to provide exceptional medical care for the patients’ and meet their whole body needs from physical and emotional to spiritual.

With the help of the local orthopedic surgeon, Adelino Rodriguez, the patients are followed through the entire process.  “It is truly an amazing gift you have helped provide. ” said Dr. John Barrington

Here is the story of one of the recent patients.  Carlos, a 42 year old working man, who suffered a femoral neck fracture in 2014 and he had no access to the appropriate medical care.  This caused his original fracture to become a non-union fracture which in turn caused the femoral Femoral Headhead to essentially disappear.  He was unable to work, and told us he only wanted to be able to walk and provide for his family.  Well after a year and a half of waiting, Carlos received his new hip!  He and his wife were overwhelmingly thankful and asked us to share his story as a way of saying, “Thank you!”

Medical Mission Trip 2016Each life has a singular value and a similarly important story.  We are proud to say that because of Move Missions, Dr. John Barrington and a number of other important people, we are able to provide Carlos a new hip and a new chance at life.  We are proud to share these stories and the ability to provide medical care to people all over the world.

Cervical Artificial Disc Replacement

Your cervical spine is made up of seven bones, called cervical vertebrae, stacked on top of each other in your neck area. In between your vertebrae are cervical discs. Your cervical discs act as cushions for your vertebrae. Dallas Orthopedic surgeon Dr. Cameron Carmody says, “Cervical discs are a marvel of the human body, they are amazing in their ability to absorb a large compressive load while also providing the range of motion between the bones in the neck.”Headshot CNC 2015

Cervical disk replacement surgery involves removing a diseased or damaged cervical disk and replacing it with an artificial disk. When the space between your vertebrae has become too narrow and part of your vertebrae or your cervical disk is pressing on your spinal cord or spinal nerves it can cause weakness, numbness, or pain. When these symptoms do not respond to nonsurgical types of treatment, disc surgery may be recommended.

In the past, with traditional cervical disc surgery, the diseased disc is removed and your cervical vertebrae above and below the disc are fused together. The FDA has recently approved the use of an artificial disc to replace your natural cervical disc. This new procedure will allow more movement following surgery and place less stress on your remaining vertebrae.

The loss of space between your cervical vertebrae could result from cervical disc degeneration, or wear and tear. Cervical discs begin to bulge and collapse with age, beginning at age 60 for most people. Some have more symptoms than others from cervical disc degeneration.

Symptoms of cervical disc degeneration include:

  • Headache
  • Neck stiffness
  • Pain that travels from the neck into the shoulders and arms
  • Numbness in the arms
  • Neck pain
  • Neurological symptoms
